Appointments

What types of appointments do you offer?

We offer both primary care appointments and functional medicine/advanced appointments. The type of appointment is determined by your intake form responses and our practitioners’ clinical expertise based on the complexity of your health needs.

Do you offer virtual appointments?

Yes, we offer both in-person and virtual appointments.

What payment methods do you accept?

We accept check, cash, credit card, CareCredit, and HSA/FSA cards.

What's the difference between fee-for-service and membership?

Fee-for-service means you pay per appointment as you need care. Membership gives you a set number of visits per year, direct access to your practitioner via our patient portal, and discounts on specialty labs, supplements, and medical services.

How do I schedule an appointment?

Start by filling out our new patient intake form. Complete the form as thoroughly as you can, and our team will be in touch with next steps. If you have questions while filling out the form, don’t worry-just do your best, and we’ll help clarify anything during your appointment scheduling.


What's your cancellation policy?

New patients must cancel or reschedule at least 48 business hours in advance. Established patients need to give at least 24 hours notice. There is a $50 fee for no-shows or late cancellations.

Do you accept insurance?

We are an out-of-network practice and do not bill insurance directly. However, you can use your insurance for labs, imaging, and prescriptions at local pharmacies and labs.


Can I use my HSA or FSA?

Yes, we accept HSA and FSA cards for payment.


What’s Included

What exactly is included in my appointment fee?

Your appointment fee covers the practitioner visit time only. Additional items like supplements, IV therapy, and other medical modalities are billed separately.

What should I bring to my first appointment?

Please bring any prior lab results, a list of current medications, and a list of supplements you’re taking.

Are prescriptions included?

No, prescriptions are not included in appointment fees. However, you can use your insurance when we send prescriptions to local pharmacies.

How do I access the patient portal?

You can log in to the patient portal at optimantra.com.

Are labs and specialty testing included in my appointment price?

No, labs and specialty testing are billed separately.

Membership

What's included in the Advanced Membership?

Advanced Membership ($200/month) includes 2-20 appointments per year based on your medical complexity, direct access to your practitioner through our patient portal, and discounts on specialty labs, supplements, and medical modalities.

How many appointments do I get with membership?

Advanced Membership includes 2-20 appointments per year, and Basic Membership includes 10-15 visits per year. The number of visits you receive is based on the complexity of your medical care and is determined by your practitioner.

Can I cancel my membership?

Memberships require a one-year commitment. It took time to get sick, so it takes time to heal. After your first year, you can switch to fee-for-service appointments anytime. If you need to cancel before your year is up, there is a $280 early termination fee. We’ve made exceptions in certain circumstances, such as job loss.

What's included in the Basic Membership?

Basic Membership ($150/month) includes 10-15 visits per year, direct access to your practitioner through our patient portal, and discounts on specialty labs, supplements, and medical modalities.

What happens if I need more appointments than my membership includes?

We work with you to ensure you get the care you need. Discuss your needs with your practitioner to determine the best plan.

What is the Biomedical Hormone Program?

The Biomedical Hormone Program is specialized care for patients needing hormone-focused treatment. After your initial consultation, program visits are $200 per visit and include 4 visits per year, plus access to your practitioner via our patient portal.

How do I know if membership is right for me?

Membership works best if you need ongoing care, have complex health needs, or want regular access to functional medicine support. If you typically need only 1-2 appointments per year, fee-for-service may be more cost-effective.

Can I add family members to my membership?

Yes. You can add a second family member for $240/month total, and each additional family member is +$60/month.
